I'm here to help try to get you what you REALLY want under the tree this year – and your part is pretty simple.  Just fill out this PDF form and return it to me by no later than November 12th.  (Although the sooner, the better.) 

Then I'll get in touch with the people you've designated, and drop a not-so-subtle "hint", so to speak – letting 'em know exactly what you've got on your wishlist, and what they can do about it if they'd like to!  And I'll monitor things to make sure you don't end up with duplicates, too.
